Services We Provide
Utilizing a person centered approach, Cross Plains Community Partner, Inc (CPCP), believes that services and supports are imbedded in individual choice, self-determination, and human rights. Central to our services is the belief that all people should be afforded an opportunity to pursue personal quality of life, and be a participating member of their community.
Community Access
This service is a facility and community based service that supports people with disabilities to have a meaningful day and achieve personal life dreams and desires. These desires are attained through exploring new opportunities that facilitate community connections, friendships and assist people to be contributing members of their community.
Supported Employment
This service provides supports that assist individuals with disabilities to find and maintain community employment. This includes job development through activities such as resume building, exploring skills and interests, filling out applications and interview preparation. Upon employment, Cross Plains Community Partner, Inc provides job coaching and support for the person to maintain successful employment.
Work Crews
This service provides the opportunity to work with a job coach who works alongside the crew to provide support.
Support Services
This service provides support to individuals receiving services in the area of human rights, self advocacy, person centered planning, health, safety and welfare.
